Transition Management provides relocation assistance and intensive service coordination to residents of qualified institutions allowing transition to a qualified community residence in a setting of their choice.

A New Hope’s Transition Managers provide additional oversight and coordination of activities for eligible participants during a transitional period for up to 12 months following a return to the community.
